Submitting Required Documents

After receiving your I-20 please complete and submit the Caldwell University health form and supporting immunization documents to the Office of Health Services. You must submit your health form to the Health Services portal for an initial review. Final clearance will be granted once the original documents are received and approved.  Original documents can be mailed to the office or submitted to the Office of Health Services upon your arrival to campus.  All forms must be complete and signed by your treating physician.  Students who plan to live on campus will not be permitted to move into campus housing until cleared by Health Services.  Students who are out of compliance will have a registration hold placed on their account which will prevent them from registering for additional semesters. 

All records will be verified for authenticity.  Fraudulent or altered documents will result in disciplinary action and a registration hold. 

You must meet all requirements outlined on the Health Form prior to your arrival at the University.

Completed health forms must be uploaded to the Health Services Portal for initial review; final clearance will be granted once the original documents are received and approved. 

Steps to completing the health form:

1. Download the Required health forms and have it completed by your doctor.  
2. Attach all laboratory results and copies of your immunization records.  
3. Upload the forms and laboratory reports to the Health Services Portal by the due date listed on the health form.
4. Bring the originals of all documents with you and submit them to Health Services upon your arrival on campus for verification.
