• Core Waiver Form
  Returning students as of Fall 2011 have the option of waiving up to six credits towards their core
• Request for Waiver of Requirements
  Do you have a Degree Requirement that has been changed? Fill out the “Request for Waiver of Requirements” form.
• Approval for an Independent Study or Tutorial course
  Would you like to do an Independent Study or Tutorial course? Fill out the "Approval For An Independent Study/Tutorial" form.
• Approval for a Temporary Grade of Incomplete
  Do you need to request a grade of incomplete? Fill out the “Approval For Incomplete Grade” form.
• Approval for a course at another institution
  Do you have to take a course at another institution? Fill out the “Approval To Take A Course At Another Institution” form.
• Change of Advisor
  Has your Advisor been changed? Fill out the “Change of Advisor” form.
• Certificate/Minor/Concentration
  Do you want to get credit for completing an Undergraduate Certificate Program/Minor/Concentration? Fill out the “Certificate/Minor/Concentration Completion” Form.
• Declaration of Added/Deleted Major
  Are you adding or deleting a major? Fill out the “Added/Deleted Major” form.
• Undergraduate Division Graduate Degree Check Acknowledgement
• Approval for Traditional Undergraduate Student to Enroll in Less than 12 Credits
